Software for somnological studies "Encephalan-PSG"

Polysomnographic studies (PSG) are carried out to study patient’s sleep structure, to define sleep-wakefulness cycles disorders, and also diseases and disorders, occurred, primarily, in sleep. PSG study allows defining and quantitively evaluating respiratory and movement disorders (chronical obstructive diseases of airways and lungs, central apnoea, restless legs syndrome, etc.), disorders in cardiovascular system activity (heart rhythm disorders, ischemic disorders), and also presence and intensity of epileptiform activity in sleep. During PSG study you can continuously record electroencephalogram (from 2 to 20 derivations), EOG (by 2 derivations), EMG (one or more derivations), ECG (1 or 3 derivations), respiration parameters (abdominal and thoracal respiratory effort, respiration airflows by oral and/or oronasal sensors), snore, movement activity (wireless movement sensors and/or EMG sensors attached onto corresponding muscles groups), oxygen saturation and other parameters at the option of user. Recorded data are analyzed to build hypnogram, to define characteristic evidences of various diseases, disorders and states manifestations. On the basis of these data, the application forms the report according to the standards and normatives, generally accepted in somnology. The report forms are the data in table or graph form or in the form of verbal description, which found the basis for conclusion making and diagnosing.

    Functional capabilities:

  • Continuous record of electroencephalogram (from 2 to 20 derivations), EOG (by 2 derivations), EMG (1 or 3 derivations), ECG (1 or 3 derivations), respiration airflow (thermistor and manometrical respiration airflows from oral and/or oronasal sensors), respiratory efforts (2 sensors), snore, movement activity (wireless sensors of movement activity and/or EMG from corresponding muscles groups), oxygen saturation and other parameters depending on delivery set.

  • Study carrying out in autonomous or telemetric mode. In the first case, physiological data record is carried out onto integrated memory card of the device. In the second case (telemetric/autonomously telemetric PSG study), data record is carried out at workstation computer (PC) via telemetric channel (Bluetooth) between patient’s transceiver-recorder and interface unit. In any of two study variants, you have an option to use voice event marker (dictaphone).

  • Synchronous with EEG record of video and audio data (EEG/PSG videomonitoring) during telemetric study.

  • Real-time control of electrode impedances and potentials during EEG record.

  • Manual and automatic (by original algorithm, using elements of fuzzy logic and cluster analysis with possibility of learning by example) hypnogram building with opportunity of editing of automatically built hypnogram and its comparing with manually built hypnogram.

  • Detection and colour display on the signal window of various types of respiratory disorders (central/obstructive/combined apnoea, hypopnea, desaturation), various patterns of encephalic activity, which allow defining sleep stages (sleep spindles, К-complexes, saw-toothed waves), epileptiform activity evidences, movements (periodic and single limbs movements) and other sleep events. Sleep events are displayed compressed in PSG window, that allows evaluating distribution of manifestations of one or another physiological parameter at length. Automatic search of some sleep events (apnoea, snore, desaturation, eye movements). Automatic marking and elimination of influence of oculogram, cardiogram and myogram on electroencephalogram.

  • Simultaneous display of dynamics of physiological parameters change with hypnogram. Indices trends of encephalic activity rhythms are shown by EEG signals, trends of respiration rate and depth – by respiration signals, HR trend – by ECG signal, SpO2 and PR trends – by pulse oximeter signal, trends of parameters of muscles tonus – by EMG signals, etc. These parameters trends are used for visual evaluation and more reliable forming of expert hypnogram.

  • Displaying of dynamics of physiological parameters change (indices of encephalic activity rhythms, parameters of respiration and cardiovascular system activity, etc.) in the form of trends for doctor’s visual evaluation.

  • Used to analyze signals of different variants of spectral analysis and topographical mapping.

  • Automatic calculation of sleep statistical parameters on the basis of built hypnogram and marked sleep events. Forming of report forms, containing data in tabular and graphic forms according to the standards and normatives, generally accepted in somnology, data on sleep stages distribution, respiratory disorders, oxygen saturation dynamics, movement activity in sleep, and also on forming of verbal description of polysomnographic study with defined evidences of various disorders to be printed.

    Notes:

  • 10 modification of electroencephalograph-analyzer “Encephalan 131-03” additionally allows recording of 6 REG channels, there is a unique option to carry out EEG-studies simultaneously with brain circulation study (REG).

  • There is an option of simultaneous record of respiration airflow by temperature and/or by pressure.

  • Wireless body position sensor is fixed on patient’s chest with belt, and it provides data record on patient’s body position.

  • Wireless movement sensor provides record of data on legs movements to detect periodic limbs movements – PLM (at least 2 sensors required) or record of movement of other body parts depending on attachment purpose and place.

  • Wireless module “Poly-4” in PLM mode provides more exact data record on legs movements as compared with wireless sensors, to detect periodic limbs movements with two EMG sensors and two wired accelerometrical sensors for movements record.

  • “PG-ECG” module provides synchronous with EEG record of ECG parameters by 3 channels and rheopneumograms by 1 channel for visual analysis of cardiorespiratory disorders during PSG-studies.

  • Wireless respiratory module provides synchronous with EEG signals record by four channels from respiratory sensors (for respiratory disorders record).

  • Pulse oximeter in set with digit sensor (standard, hard, medium-sized) provides synchronous with EEG record of degree of oxygen saturation of patient’s aerated blood. It can be supplied with additional sensors or sensors of other types on request.
